Age-Friendly Collaborative Expands in US – Why not Here?

Thoroughly impressed that 30 US health systems are collaborating on implementing #agefriendlycare which puts the #4Ms at the heart of their actions: #WhatMatters, #Medication, #Mentation and #Mobility. Can a similar number in our country of province commit to doing the same?

We need to commit to this: every older adult should receive evidence-based, age-friendly care that focuses on what matters most to them!
