US Hospitals not capturing/acting on half of patient harm events

(USDH&HS) In the US, the Office of the Inspector General found that in 2022 𝗵𝗼𝘀𝗽𝗶𝘁𝗮𝗹𝘀 𝗱𝗶𝗱 𝗻𝗼𝘁 𝗰𝗮𝗽𝘁𝘂𝗿𝗲 𝗵𝗮𝗹𝗳 𝗼𝗳 #𝗽𝗮𝘁𝗶𝗲𝗻𝘁𝗵𝗮𝗿𝗺𝗲𝘃𝗲𝗻𝘁𝘀 𝗶𝗻 𝗠𝗲𝗱𝗶𝗰𝗮𝘁𝗲 𝗽𝗮𝘁𝗶𝗲𝗻𝘁𝘀. Of the events captured, 𝗳𝗲𝘄 𝘄𝗲𝗿𝗲 𝗶𝗻𝘃𝗲𝘀𝘁𝗶𝗴𝗮𝘁𝗲𝗱 and 𝗲𝘃𝗲𝗻 𝗳𝗲𝘄𝗲𝗿 𝗹𝗲𝗱 𝘁𝗼 𝗽𝗮𝘁𝗶𝗲𝗻𝘁 𝘀𝗮𝗳𝗲𝘁𝘆 𝗶𝗺𝗽𝗿𝗼𝘃𝗲𝗺𝗲𝗻𝘁𝘀.
